Easy Read now available: Media Permission Forms

A new Easy Read is now available for Media Permission Forms, which was developed as part of our NDIS recertification to ensure best practice for our customers.

This is the form that needs to be signed by customers or their guardians if they wish – or do not wish – to participate in any form of media, including online media, broadcast, print news, and our own brochures. Basically, that’s any time they are likely to be photographed or filmed and the photos or film may be published.

This new Easy Read form (QF 3107.02) replaces the old media permission form (QF 3107.01).

We recommend that media permission forms are renewed annually and for major events, for example Battle of the Bands.

Is there a version of this new form that is not in Easy Read?

No, there isn’t. This is the new form.

Does this mean the old forms are now invalid?
No, the old forms will still be valid. However, we recommend having the new form signed annually.

If a customer is participating in media or marketing before the form is set for annual renewal, you should have this new form signed ahead of participation.

Why sign them annually?

From a legal perspective, they are signed “in perpetuity”.

However, it’s good practice to keep them up to date as a customer’s needs or views may change over the years and we all have the right to change our minds.

What if a customer or family member changes their mind?

If a customer or their decision-maker decides they do not wish to participate in media any more, they can simply fill out the appropriate section of this same form.

If the customer has previously participated in a photoshoot or you believe Endeavour Foundation may have their image on file, please contact the Media Advisor and Head of Marketing, who will ensure the images are removed.

Do I need to do anything else?

Please keep the new and old media permission forms on file and refer to them if participating in media or an event where a customer is likely to be photographed or filmed.

It is important to have all the forms on file, including the old forms for customers who have changed their minds.
