Home and Community Frontline Leaders are invited to register and attend an information session about the Home & Community Annual Client Impact Survey.
A client impact survey has been developed to be completed by people we support in Home and Community settings with support from frontline employees between Friday 6 December and Sunday 15 December 2024.
People we support who are part of multiple services delivered by Home and Community will complete a survey at each service they attend with support from frontline employees.
Our Strategic Plan prioritises five key outcomes that are important to the people we support – feeling safe, being healthy, empowered to make decisions, included in their community and having a paid job that is right for them.
In December 2024, we are asking people we support in Home and Community how they feel against the five outcomes to establish a baseline that we can measure against annually.
We want to know how people feel against these outcome measures because we’ll know we are making a difference when we see positive improvements in our outcomes.
If you have any further questions about the client impact survey, please contact Head of Practice, Quality and Impact, Kate Martin by email: [email protected].
