
Step One: Understand Support Needs
The Enquiry Form (over 18 years) and Enquiry Form (under 18 years) captures essential information to understand the support needs of those interested in Endeavour Foundation services. It helps the Intake and Assessment team (or local team if contacted directly) identify relevant supports.
- Complete the form during the initial phone call, if possible.
- Submit within two business days of receiving the enquiry.
- If completed by the local team, send it to the Intake and Assessment team at [email protected] for tracking in Salesforce and regular meetings.

Step Two: Connect with Local Team Tour
Our goal is to quickly connect new clients with local teams and allow them to tour our services before sharing detailed information.
Prospective clients can learn about our services and meet the local team. Schedule any tours or meetings during the first phone call, with assistance from the Engagement Coordinator. Clients can choose to have the NDIS team join the tour and provide a service quote.
Review details from the Intake and Assessment team to know what’s needed. You can use the checklists provided to prepare for this step.
During the tour, remember to collect any required paperwork. The Intake and Assessment team will inform you of outstanding documents. Prepare any necessary paperwork for signatures to avoid delays in starting their service.

Step Three: Assessments of Supports Form
This Assessment of Strengths, Needs, and Supports Form (QF 1329.01) helps local teams gather essential information about a person’s support needs and how Endeavour Foundation can assist them.
It should be completed during the tour/meeting in a conversational way, encouraging questions to ensure a smooth transition. Refer to the Assessment and Transition Procedure in the resources section below for more details.
The Practice, Quality, and Impact team is available for support and guidance as needed.

Step Four: Approvals
People that are identified as not needing a support profile (as per the Assessment of Strengths, Needs and Supports Form), can be immediately onboarded with Operational Manager approval. Notify the Intake and Assessment team of any new people who can commence immediate onboarding.
People who need a support profile will be referred to the Practice, Quality and Impact team for further review and recommendation of support interventions prior to approval to transition.
This is about finding ways to support people to successfully and safely access our services. People that need a support profile, or any person who is determined not suitable for Endeavour Foundation services, will require General Manager (GM) approval to proceed.

Step Five: Welcoming to Our Services
Once a new client is ready to start services, the Intake and Assessment team (or local team) will contact them to confirm their start date and introduce the local service delivery team and NDIS Coordinator.
After onboarding, the Intake and Assessment team informs the NDIS team to finalise paperwork. The NDIS team will send an introduction email with relevant documents, set up the client in Carelink, and forward information to billing for immediate invoicing. No signature is needed to start services and billing.

FAQs
What if the person making an enquiry doesn’t have funding?
The Intake and Assessment team can help people understand the steps they need to take to obtain relevant funding. Email templates have been developed to support people to take these steps. Regular contact is made by the Intake and Assessment team throughout to continue to nurture the enquiry.
What if an enquiry is for a new build under construction?
When someone enquires about a new home under construction they go on an exciting journey learning about construction updates and housemates. The Intake and Assessment team will regularly engage with people over this period. Anyone waiting on a new build will now receive My Home My Life construction updates with exciting news/photos etc, as well as invitations to Endeavour Foundation events. When it gets close to opening we will support local teams to establish relationships and introduce potential housemates.
What if there is no suitable Endeavour Foundation vacancy?
