The support we provide may sound a little complex, so let’s break it down for you.
Our multidisciplinary team are experts in disability services, positive behaviour support, social work, and practice improvement. We champion the right for people with disability to exercise choice and control, and have access to individualised support.
We can help you tailor supports to individuals with complex needs or circumstances, whilst ensuring the highest level of disability practice is achieved and maintained.
For complex support, please complete our QF 8700.03 Complex Support Referral Form and obtain approval from your Operations Manager. Once approved, email the form to [email protected].

What We Do
- Provide supports for clients impacted by life events (e.g. those experiencing grief and loss, end-of-life care, hospital discharge, or unhealthy relationships), and access to external services specialising in domestic and family violence, sexual assault, mental health, advocacy and counselling.
- Assess individuals displaying concerning behaviours to develop appropriate strategies that reduce risk and improve safety.
- Complete psychosocial assessments to inform guardianship applications.
- Provide advice and lead all reporting on the use of restrictive practices.
- Liaise with allied health practitioners, including general practitioners, occupational therapists, psychologists, dieticians, and physiotherapists, to facilitate holistic care for clients.
- Assess individual support needs as part of the My Home, My Life initiative.
- Provide staff with advice on assessing risk and practical disability support resources to improve client safety and wellbeing.
- Coordinate all reporting to the NDIS Quality and Safeguards Commission and Department of Communities, Disability Services and Seniors (DCDSS) regarding restrictive practice.
